Ultracode is decoupled from effort

In earlier versions, Ultracode was selected as the top position on the /effort slider and forced xhigh effort. Claude Code 2.1.284 makes it an independent toggle. Users switch it with Tab in the /effort picker or with /effort ultracode [on|off], and it stays on at any effort level.

Keybindings

The release also adds effortSlider:decreaseEffort, increaseEffort and toggleUltracode keybinding actions, so the slider's arrow and Tab keys can be rebound in keybindings.json.

VS Code

The VS Code extension mirrors the change with an Ultracode on/off switch under the Effort slider, replacing the slider's Ultracode stop. The model pill shows "路 Ultracode" whenever it is on, regardless of the effort level.
